All student participants are required to have an up-to-date athletic physical on file at their school (valid for 36 months).  Please understand that an athletic physical is different than a well child exam.  Minnesota State High School Athletic Physical Forms are located on the MSHSL Health and Safety Page.  Athletic participation physicals are available with your primary care physician, the School Based Clinic (Free), TRIA Orthopedic Clinics on select dates (Free), and Twin Cities Orthopedics on select dates (Free).
